A vacation nearly turned tragic due to acute myocardial infarction
While en route to the airport, Mr. L. Bobby suddenly experienced severe pain in his left chest, radiating to his shoulder, accompanied by a sensation of chest tightness and shortness of breath. Initially, he was taken to a local medical facility, but the test results were inconclusive and unable to confirm an accurate diagnosis.
As soon as he was transferred to Hong Ngoc General Hospital, a quick evaluation revealed a history of hypertension, diabetes, and lipid disorders. These are underlying conditions that significantly increase the risk of myocardial infarction. Doctors promptly conducted an electrocardiogram (ECG) and ordered the necessary laboratory tests for further evaluation. With a high level of clinical vigilance and caution in approaching a patient presenting with classic symptoms and a significantly elevated Troponin T level (253), the medical team decided to perform multiple electrocardiograms (ECGs) to monitor dynamic changes and record electrical activity from different leads, aiming to identify potential areas of myocardial injury.
The electrocardiogram results revealed ST-segment elevation, indicating abnormalities in the inferoposterior leads and right ventricular leads. This was a critical diagnosis, indicating that the patient was at high risk of experiencing an acute myocardial infarction, which could be life-threatening if not treated promptly.
Rapid emergency response – a race against time
Recognizing the critical condition, the medical team promptly activated the hospital’s acute myocardial infarction emergency protocol. The patient underwent coronary angiography using an advanced Digital Subtraction Angiography (DSA) system, the imaging revealed severe stenosis in segment II of the right coronary artery, one of the heart’s main blood vessels. To restore blood flow and prevent further cardiac damage, the doctors decided to place a stent at the site of the blockage.
However, an unexpected challenge arose during the procedure: while performing the intervention via the radial artery, the patient experienced severe arterial spasm, making stent placement technically difficult. Thanks to their sharp clinical judgment and extensive experience, the medical team quickly switched to the left radial artery approach and successfully restored blood flow, helping the patient escape a life-threatening situation within moments. In addition, the doctors employed a technique to protect the right ventricular branch, ensuring effective reperfusion and optimizing post-intervention outcomes.

Persistent efforts – For the patient’s survival
When informed of his heart attack diagnosis, Mr. L. Bobby was understandably distressed and initially wished to return to his home country to continue medical care. However, driven by a deep sense of responsibility and a patient-first philosophy, the doctors at Hong Ngoc General Hospital persistently counseled the patient, carefully explaining the high risks associated with delayed intervention. “Time is Life” – This is the golden rule in the emergency management of myocardial infarction.
